Shopping cart error - can not remove item

Check this out...I can not replicate this problem, but have heard from a few of our customers.  They can not remove items from their shopping cart and it seems to keep adding new rows for the same item instead of updating quantities.

In short, please note the absence of the "x" to remove the item.

Many thanks in advance for any advice.
