User Event Script Skipping Records

Since our upgrade to 2013.2 in Sept., we've been having an issue with a user event script completely skipping records.

We drop ship all orders. We have workflows to approve most SO's if the order meets certain criteria.  If the SO doesn't meet the criteria, the SO is manually approved.  Approving the SO creates the PO and then the user event script is triggered to send the PO to the vendor. 

PO records are randomly being skipped by the user event script when the orders are manually approved.  Not all PO's are skipped, only some.  It seems to occur when SO's are being approved manually around the same time workflows are approving SO's.
