Hundreds of Time Entries Unapproved

On January 16, we had hundreds of time entries (timebill) -- that had been previously approved -- become unapproved.  Many of the time entries were already billed, but they still were set back to unapproved.

Has anyone else had this issue? 

So far we have not been able to identify any pattern in the time entries, projects, and project tasks.  We have no workflows or scripts that set Approved field (supervisorapproval).

Without system notes on time entries, forensics is challenging.
