Custom form possibility???

I'd like to be able to create a credit application template on the customer contact page to be utilized by the sales department. The customer info would be filled in automatically by the system similar to the quotation, sales, invoice forms. The sales rep would fill in all the addl pertinant info and then send the form to the customer by email.

is this possible?????
