Online form throwing an error

I have created an online form to create a lead.  I have server scripts deployed to lead records and everything works fine from a logged in session.

When I submit the online form externally, I get this error:

Permission Violation: You need the 'Lists -> Employees' permission to access this page. Please contact your account administrator.


I have ensured that my script file has the "Available without login" option checked.  I don't see anything else that would require changing.

I also would like it so that if a user doesn't find the record they want in a List/Record field they can add a new entry.  Is this possible with Online Forms?
