Problem: Address fields in online case form-Please help

Does anybody has the problem of getting address fields populated for online case forms. When I add these fields to online case form and fill in the fields I do not see these fields getting populated in customer record or case record. I have opened a case with NS, but I think it is going to take months to resolve it. I hope I can get a quick answer from the group here. Thankyou
