HTML version of Advanced PDF/HTML Packing Slip

We are using the Advanced PDF/HTML for Packing Slips.  For some reason the PDF of the packing slip cannot be emailed from the Item Fulfillment, we get an error message (an unexpected error has occured).  Rather than file a support case (we are self service) I am trying to work around this. 

When I select to send the HTML, or In Line versions of the form instead of the PDF I get a form that has ALL of the information from the transaction (this includes Field Names for fields that are not even exposed on the Sales Order form and have no data).  Anyone have any insight on this? Is there any way to modify the HTML version of the Advanced PDF/HTML form?
