Auto Responder Emails not Working

Our Auto-Responder emails are not working.  We believe that it started Version 11.  A prospect comes to our web site, fills out a form that is passed to our application.  Our application fills out the NetSuite form, which then creates a Prospect record and populates the record with the correct information.  However, it does not send out the email like it used to.

Is anyone else experiencing this?
