Transaction Uploads

I am attempting to build a transaction upload to load Opportunities for my sales reps.  So far I can upload the file and create a Opportunity but I am not getting any items.  I have designed my template to include all required fields but still no luck. 

I am hopeing to find someone that has got this to work and can share the template that you used.
