Workflow manager... assign task to current record supervisor

Trying to assign a task to the current records supervisor (it's for an employee provisioning workflow)

Trying to get the task title to be along these lines;

Title = please ensure that "employee name" has returned employment contract paperwork

To get this to happen should I be using a formula? if so, what type?

I can get the employees name into the title by putting Name/ID as the value field in the title line but how can I join the text??
