Packing Slip - 1/2 blank (no items or descriptions)

We just had the upgrade done over the weekend.  In the testing I had no problems, but now that I am live, I no longer have functioning packing slips. 

We use custom forms, so I thought something changed, reverted them all back to the standards as a test, still same issue.

Anyone?  Ideas or suggestions?  Hard to supply a packing slip that is 1/2 empty.

I get our corporate information, customer’s bill / ship to address, a row with some details like terms and such, message field at the bottom, then nothing showing the items or qty or description.
