Duplicate Invoice Numbers without Warning

From time-to-time we are still getting invoice numbers duplicated without warning. Since we are a small company with only 5 NetSuite users so I would presume that companies with many more users would be seeing this more often? Is anyone else experiencing this? We are trying to gather more information about the conditions when this is happening.
