Create a custom packing slip template

**  I posted this in the wrong forum, apologies.  I would delete it if I could.  :0(

We drop ship for some of our distributors to their customers.  Our distributors would like a packing slip with their information (address, company log, etc) to be sent with the shipment.

I'm looking at the the standard packing slip and I don't see how I can change the company logo, address, etc.  I think what I need is an all new, custom template with the company information hard coded into the PDF.

Is there any way to accomplish this?  To create a new Packing Slip form that can show a specific address and company name?
