Packing Slips and Return Authorizations

This one struck me as odd.  Today I noticed that a Packing Slip for -2 printed from the Que.  It was for a Sales Order that had just been issued a Return Authorization for Qty 2.  Why is a Packing Slip printing for a Return Authorization?

My shippers misread the Packing Slips -2 for a 2 and thought they had lost the picking ticket for an order.  They proceeded to reprint the UPS label directly from the original Sales Order and ship out the product.  Not good for a client when they get two more of something they were trying to return in the first place.  Any ideas?
