Missing Fulfilment- hence stock error.

I have the strangest thing.

We have an item of stock where Netsuite is showing 25 in stock. However this is not true and when I look at the history I can see the following
1. Sales Order
2. Purchase Order
3. Item Receipt
4. Cash Sale
5. Supplier Invoice.

But no fulfilment. How can the fulfilment be missing? I have experimented and if I delete a fulfilment after billing the customer the system is smart enough to prompt me to fulfil the order again. I am not being prompted to fulfil the order so I am at a loss to know how this has happened. I need to re-instate the fulfilment so that the stock is correct and the balance sheet balances. How can I identify other transactions where this problem may have occurred?
