Choosing which Packing Slip form to use

We've got a situation where we need to use a custom packing slip when shipping orders for one customer.

I've got the custom packing slip form made, the custom pdf layout made and assigned to said packing slip, but I'm frustrated that there is no easy way to choose which packing slip to use when completing the item fulfillment form.

I've read the help files pertaining to packing slips, and I can't find any answers there.

Am I missing something?
