Developer help needed - Packing Slip Design Work

We just got our permit for the US Postal System's Merchandise Return Service. This should allow our customers to affix a label on a box back to us, and we are only charged for items shipped. The packages are sent to a local Postage Due Unit where they are sorted, we are billed, and they are delivered back to us to process as returns.

We need someone who is familiar with designing custom PDF forms within Netsuite so that we can use the bottom half of our packing slips for the USPS label. The specifications can be found on the USPS site here:
