Mysterious Shipping Issues?

Since 04/12/08, we have received complaints from customers regarding similar but somewhat different shipping problems.

Problem 1: No shipping option available for an address (residential address within U.S., which we DO ship to.)

Problem 2: Shipping is not properly calculated and althought the shipping cost is not calculated on the confirmation email to customer, the shipping cost shows up on the system!

Anyone has the same problem?? Was Netsuite doing system update?? We were unable to duplicate the problem internally, but keeps getting emails/calls from customers about the same issue.   

PLEASE HELP!!!!
