Not autopopulating shipto address from web orders

Orders coming from the web are occasionally not auto-populating the Ship To address once the order goes to the Fulfillment stage.  The Sales Order will have Ship To and Bill To both populated normally, but the Ship To is lost at fulfillment and has to be populated from the drop down box on the Address tab in the Fulfillment screen.  There do not seem to be any format issues with these addresses, they just don’t show up.

Anyone seen this or have any ideas?
