Duplicate out put for a saved search

We recently created a new transaction search.  We had 3 filters:  transaction type of Invoice and credit memo, account does not equal the AR account, and we excluded some items that were memo fields.

For some reason we are getting some records returning 2 or 3 times.  It's not all records, just some of them.

Has anyone had this issue in the past?
