How do I write to a custom field in Item List?

Is there a way to write to a custom field in VB.net? or even C# for that matter? I have seen some posts but none seem to work.
Here is some code that in the IDE shows the custom field being populated but as usual, throws and error when requesting an update or ad.
Dim ifn(1) As ItemFulfillment ' instantiate an ItemFulfillment
ifn(0) = New ItemFulfillment
ifn(0).internalId = myRefNum 'Just returns a blank ItemFullfillment
ifn(0) = myList(0) ‘ so i add item fullfilment from an earlier search...
Dim strCF As New StringCustomFieldRef
strCF = New StringCustomFieldRef
