retrieve item with search criteria of custom field

Hey guys,

I'm having some real trouble on this one.  I'm working on an issue that requires an update of an item in an itemlist for a cash sale.  The problem I'm having with this is that the search criteria to retreive the item I need to update is a custom field.  Once I have the item, its a piece of cake, but I can't seem to do anything with customFieldList to bring back the item I want.

Does anyone know how I can do this?
