Adding Custom Record with PHPToolkit

I'm trying to use the PHPToolkit (wrapped in a kohana module http://github.com/iloveitaly/netsuite-kohana) to add a custom record.

If I specify the name field in the array passed to setFields I get a INSUFFICIENT_PERMISSION error. If I do not specify the name field I get the following message resulting from a SSS_INVALID_SRCH_OPERATOR error:
[CODE]An nlobjSearchFilter contains an invalid operator, or is not in proper syntax: name.[/CODE]

I found someone who apparently got this to work (https://usergroup.netsuite.com/users/showthread.php?t=20368) but using nearly identical code I am getting the INSUFFICIENT_PERMISSION error.
