web service creating duplicate sales order entries

I have a web service application that has been in use for 3 years now.  This application creates new sales orders.  It has been working correctly until recently.  Without any changes to the code, the sales orders are being duplicated in NetSuite.  According to the web services usage log the duplicate record is created within 30 seconds of the original record and is the same as the original record except a different user account/login is listed as the creator on the duplicate.  Has anyone else experienced a similar problem? 
The application is a stand-alone windows application that is installed on several different computers with each computer having a separate login.
