HELP! download with form crashing

I've just built my first Download with Form. Nothing fancy afaik.

The form and download test out fine, seperately, but when linked together, the form crashes on submission with the message "An unexpected error has occurred. Technical Support has been alerted to this problem." (It doesn't alert us, though...)

I've also tried different forms. No record is created/updated before the crash message (creates fine with just the form). The pdf file (c. 0.7Mb) is set to download without logon. Have tried IE and Firefox, both when logged into NS and when logged off. Have tried to vary workflow and change fields. We don't [yet] have the webstore digital download module. No scripts at this point.
