Online Case forms creating new Leads not linking to Existing Ones

Hi

I am trying to set up a link from our newsletter to an online case form.

The link is passing parameters for email address, firstname and lastname - which populates these fields on the online case form. E.g:

https://system.netsuite.com/app/crm/marketing/campaignlistener.nl?__lstr=__cl&__r=10861&c=608194&__h=5a8bd74c9e5c9cff2c87&url=https://forms.netsuite.com/app/site/crm/externalcasepage.nl?compid=608194&formid=19&h=15a14282646a094e3bf3&firstname=Test&lastname=Nicholson&email=andrew.nicholson4576@ntlworld.com&title=Coaching%20Request%20Form

I want the recepient of the newsletter to follow the link, complete the online form, submit it and have it show up as a case against their record.
