need script for multi sites- help?

I am working towards finshing up our multi site module.  I have created the custom forms for the different sites, but I need to come up with a server side script in order to pull the correct form based upon the website the order originated.  I have a simple client side script, but of course, it wont work for web orders, so it is not very helpful.

Anyone want to help me out here?
