Online form that gets filled out and emailed to us...

I have created an online form:
SETUP > MARKETING > ONLINE CUSTOMER FORMS
Here it is (don't laugh):

Right now all the info get's stored into a customer record as a Lead, which is fine.  However I want all the info to be emailed to me so I can reply to the customer and pick a bodyboard for them.  Seems easy enough and I think it you guys are going to tell me to use Suitelets which I know nothing about. 

Can anyone help me??
