Form won't work

I'm trying to create a NS form and place it in an iframe in an external site. The NS form is at https://forms.netsuite.com/app/site/crm/externalleadpage.nl?compid=575447&formid=2&h=fa79b76b932100b0eae0, and it works. The page I'm working on is at http://www.therpagroup.net/testform.html and only displays a blank box. I followed the directions in the NS help, but it's not working. Any ideas?
