Google Affiliate Network and Conversion Tracking Implementation on Shopping Cart Checkout page.

I am setting up the Google Affiliate Network and conversion tracking.  I tried searching these help forums for info on this but I came up with nothing so I thought I would start a thread about this for people to copy paste in the future from what I/we can come up with.

So far I have
