Published Customer Form showing NA

in Web Site / E-Commerce

I have created an Online Marketing Form and published it on the website. Using a Presentation Tab "Contact Us", I have displayed the form.

When I click on the "Contact Us" tab on the website, why does it show the Contact form as a link on the form itself instead of displaying the whole FORM on the contact us page and display "N/A" at the bottom in a box and you have to click through the link to view the actual form.
