PayPal Integration Vs Express Checkout

Paypal Express Checkout integration allows payment to be refunded from within Netsuite, without it, you  can't refund.
So is that the main difference?

Do the shopper need to have a PayPal account as well to put payment through the webstore?

In the shopper's workflow in the help guide, the diagram implies that if you checkout with Paypal, you need to enter PayPal login or create a new account.
I was told that there is an option to pay by Credit Card as well with Paypal integration without needing a PayPal account.

Is that true?
