Help diagnosing customer's checkout error

I have customer that is reporting getting the error "You do not have permission to perform this operation" when clicking the submit button during checkout.  I'm unable to reproduce the error on my end (created a new customer and used my own customer login).

He report that this happens in both IE and in Chrome.

Any ideas on what might cause this?
