Online Leads Being Recorded as individuals not Companies

Our company uses an online form to capture leads. Within Netsuite, we have our customers set to be setup as Companies, not Individuals. For some reason, the online form is creating these Leads as individuals.

Does anyone have any ideas why this would be happening? All the settings appear correct to have these leads setup as Companies.
