Online Form Errors

We have had an issue with one of our online forms for some time now.

I have tried time @ again to resolve it, with no luck.

The link to the form is here:

https://system.netsuite.com/app/site/crm/externalcasepage.nl?compid=400884&formid=4&h=fa095fed7682b65ed723

...and the error text is

"Invalid company reference key 1687."

Hopefully some one can assist.
