10% discount applied on a web order automatically!!!

Hi,

We have an order submitted through our website where "Discount" & "Rate" fields filled with 10% discount with empty "promotion" & "Coupon code" fields.

anyone have an idea for how this discount applied to a web order?

FYI:
* All our discount items are offline (not displayed on website)
* No price level applied in customer record
* We didn't apply this discount manually
* All our valid promotions have a coupon codes
