How to Edit Reviews Template

Hello All - We have a reviews template that is load via an iframe from this URL https://forms.netsuite.com/app/site/crm/externalcustrecordpage.nl?

I have no idea where i can edit this form at? Anyone know how this can be edited? We just need to add some text to the HTML saying that every field is a required field.
