How to add different size and quantity for single item in Item list template?

Hello,

I am working on a Netsuite website.
There is requirement like:

in the Item list template, there should be different sizes and quantity field for each size.

And, on click Add to cart button, need to add different sizes and quantity for a particular item.

How to add this?

Can anyone help me on this?
