Are you able to insert a form using getAttribute?

We have a contact form that we want to include in the item's description. So we created a an email form in Netsuite but it's like it's own page, so clients have to click on a link then go to that page, is there a way to insert the form using getAttribute tags?

What other options do we have to insert a contact form so clients can send us emails about the product directly from the item description?
