Meeting Form adn Contact USer Notes search

Has anyone out there craeted a Meeting Form to record summary of a meeting held?

related issue.  Can i create a saved search to find user notes attached to a contact withing last week with Meeting in the title?  I can do it for USer Notes attached to a Customer but not to a Contact
