How do I use radio buttons on an online form?

I have been attempting to create an online customer service survey and I want to be able to use radio buttons instead of the drop down option for a list/record field type to rate their satisfaction based on different criteria (i.e. friendliness, helpfullness, etc). I have my html template set up but I can't figure out how to pass the values selected for the radio buttons to a selection for the NetSuite field. Any suggestions?
