Journal Entry Report

Is there a way to create a report that shows all journal entries including who created the entry and who approved the entry?

I am not sure how to add these fields to the report.  I tried to use a custom field on the entry, but do not know how to set it up so that it captures this information automatically. 

Any suggestions would be greatly appreciated!
