2 Equity Sections in Balance Sheet

Hi Everyone,
We have a balance sheet that suddenly displays two Equity sections in the Balance Sheet.  They are counted as one so the numbers are correct.  However, this is very strange.

Has anyone seen or heard of this issue?  Any suggestions?
