Neat format for online case form

Hello,

I am trying without much luck to get the Incoming Message & Title to neatly align on the page.

Is there a way to force alignment on these forms?

This is my form, un-aligned... despite setting all width as identical - this behavior occurs on other form also...

Also as this case form is also going to create a new lead, must I create a custom field on cases to record email opt-in/out? Or can I insert something into the url to say "&new lead unsubscribe is false"?
