Online form in iframe, submission fails: Invalid page parameter. Unable to view page.

I'm not using NetSuite to host my site, but I do have several Online Customer Forms embedded using iframes into various pages throughout our site.

I recently noticed a drop in leads and started looking into the cause. What I've found so far is that Safari is displaying the form in the iframe as expected, but on submission the following error is displayed in the iframe, below the NetSuite logo:

"Invalid page parameter. Unable to view page."

From here, the user is stuck -- no data will post in this scenario.

Checking the browser's debugger gives us this message:

"Blocked a frame with origin "
