getting rid of record not updated prompt

I am trying to create a form that updates the customer record with some additional information.  Once the form is submitted I thought that I needed to use another form to display a thank you message for submitting the form.  My second form displays the thank you, but it also has a submit button on it that gives a screen prompt that the record has not been changed, do you want to continue.

Am I doing this wrong, or is there a way to not update with the 2nd form?
