Contact First/Last name no longer passing to new lead email notification

I have an online lead form that contains fields for first and last name. My custom sales rep notification email template for this form includes company, country, state, first/last name, etc.

This form and the sales rep email notification have been in place for years, but with the 2014.1 release the contact name info is no longer displayed in the email notification.

I know that CRMSDK is deprecated so I've started rebuilding the email notification with a 2.0/scriptable template instead, but I'm still unable to get the contact's first and last name to populate into the email notification.

Anyone else? Have you found a solution? Thanks!
