Online form error

Tried to create a simple online support case form (had one already just needed a 2nd).

Everytime I went to save it I just got
    
   
  
  
   
  
  
   
  
  

You are missing the following required field(s):
  
   CUSTEVENT1
   CUSTEVENT3


Cannot do anything :-(

Then tried to edit the one I have been using for years, and can't save anything new because get the above errors.

Anything obvious?
