Attach Custom Remittance Slip to Invoice

I want to remove the Credit Card information from my Remittance Slip.  I created a Custom Remittance Slip and un-checked the box for "Credit Card Info".  Then I made it my preferred form and saved it.  However, when I print my invoices, it is still printing the credit card info at the bottom on the Remittance Slip. 

Any idea on what I'm doing wrong?
