Purchase Order - Quick Add to Ship To disabled in 11.0.6?

We used to be able to quick add a ship to address.  The drop down would allow us to select an existing address, or select "New" or "Custom".

Seemingly, this has been disabled as of 11.0.6.  The "Bill To" address still shows New and Customer, but the "Ship To" does not.

And the Plus sign does not work, either.

Any ideas?
